• Fill out your Name, Address, Phone Number, Fax Number and Email address
• Fill out the vehicle information from your registration
• Fill out the Vehicle Identification Number or VIN
• Fill out the odometer reading
• Fill out all of the optional equipment
• Last but not least, try to be honest about the condition of your vehicle; you can use the condition grades provided on our form.

 Instructions for taking pictures of your vehicle

Take at least one picture of the the following

• One front corner of the exterior
• One rear corner of the exterior
• One of the interior
• One under the hood

We need quality pictures, not quantity.

Your pictures should look like this:

Front corner of the exterior	Rear corner of the exterior
Interior	Under the hood

Mail the form, pictures, and check for $50 to
